PYEA Student Experience Sponsorship

Have you moved?

If you have a student that attends the Penn Yan School District and have moved to a new address, please contact the school in which your child attends.  A change of residency form and proof of your new address is required in order to change it with the District.

Important Health Information (Click here for NYS Immunization Requirements)

New York State Law requires a health examination for all students when entering PreK or K, 1st, 3rd, 5th, 7th, 9th, and 11th grade. The examination must be completed by a New York State licensed physician, physician assistant or nurse practitioner.
A copy of the health examination must be provided to the school within 30 days from when your child first starts at the school, and when your child starts K, 1st, 3rd, 5th, 7th, 9th and 11th grades. If a copy is not given to the school within 30 days, the school will contact you.
If your child has an appointment for an exam during this school year that is after the first 30 days of school, please notify the Health Office with the date.
We suggest you make copies of the completed forms for your own records before sending them to the school Health Office. Forms may also be faxed to the numbers below:

Community Wide Grocery Distribution

Pre-registration is required! Register online or call 315-536-5183 and follow
prompt to receptionist. *Registration is on a first come, first served basis*
• This will be a drive-thru model; clients must stay in their car and put a piece of paper with first name and last name in window facing out for staff to see at check-in.
• Please have trunk cleaned out so box of emergency food can be put into it. Once your trunk is closed and tapped, you may drive away. This will be a no-touch distribution and proper social distancing must be maintained at all time.
• No walk-ups; only pre-registered households in cars will be served.
